Streszczenie
Przewlekła niewydolność nerek jest postępującą chorobą prowadzącą do nieodwracalnego
uszkodzenia miąższu nerek, zaburzeń filtracji kłębuszkowej oraz kumulacji toksyn mocznicowych
w organizmie. W zaawansowanych stadiach choroby konieczne jest wdrożenie leczenia
nerkozastępczego lub transplantacja nerki. Celem rozdziału było określenie zakresu opieki
pielęgniarskiej nad pacjentką z przewlekłą niewydolnością nerek po przeszczepieniu nerki.
Zastosowano metodę studium przypadku. Materiał badawczy zgromadzono na podstawie
analizy dokumentacji medycznej, obserwacji pacjentki podczas czterech wizyt domowych,
wywiadu, badania fizykalnego oraz pomiarów. W badaniu wykorzystano Kartę Opieki Pielęgniarskiej,
wskaźnik BMI, skalę oceny niedokrwistości oraz Skalę Akceptacji Choroby (AIS).
Badaną była 33-letnia kobieta po transplantacji nerki, pozostająca pod stałą opieką poradni
transplantacyjnej. W wyniku przeprowadzonej oceny stwierdzono występowanie objawów przewodnienia, zmęczenia związanego z niedokrwistością oraz ryzyko zaburzeń elektrolitowych.
Pacjentka wykazywała wysoki poziom samodzielności w zakresie samoopieki oraz
bardzo wysoką akceptację choroby (36 punktów w skali AIS), jednak zgłaszała lęk związany
z możliwością odrzucenia przeszczepu i koniecznością przewlekłego leczenia immunosupresyjnego.
Na podstawie zgromadzonych danych sformułowano diagnozy pielęgniarskie oraz
opracowano indywidualny plan opieki obejmujący działania diagnostyczne, terapeutyczne,
edukacyjne i profilaktyczne. Wnioski wskazują, że kompleksowa, zindywidualizowana
opieka pielęgniarska odgrywa istotną rolę w poprawie jakości życia pacjentów z przewlekłą
niewydolnością nerek oraz wspiera ich funkcjonowanie w środowisku domowym.
Chronic kidney disease is a progressive condition leading to irreversible damage to the renal parenchyma, impaired glomerular filtration, and the accumulation of uremic toxins in the body. In advanced stages of the disease, renal replacement therapy or kidney transplantation is necessary. The aim of this chapter was to determine the scope of nursing care for a patient with chronic kidney disease following kidney transplantation. A case study method was used. Research data were collected based on an analysis of medical records, observations of the patient during four home visits, a medical history, a physical examination, and measurements. The study utilized the Nursing Care Plan, BMI, an anemia assessment scale, and the Acceptance of Illness Scale (AIS). The subject was a 33-year-old woman who had undergone a kidney transplant and was under the ongoing care of a transplant clinic. The assessment revealed symptoms of fluid overload, fatigue associated with anemia, and a risk of electrolyte imbalances. The patient demonstrated a high level of independence in self-care and very high disease acceptance (36 points on the AIS scale), but reported anxiety related to the possibility of transplant rejection and the necessity of chronic immunosuppressive therapy. Based on the collected data, nursing diagnoses were formulated, and an individualized care plan was developed, encompassing diagnostic, therapeutic, educational, and preventive measures. The conclusions indicate that comprehensive, individualized nursing care plays a significant role in improving the quality of life of patients with chronic kidney disease and supports their functioning in the home environment.
