Streszczenie
Dna moczanowa jest przewlekłą chorobą zapalną stawów związaną z odkładaniem się
kryształów moczanu sodu u osób z hiperurykemią. Schorzenie prowadzi do nawracających
napadów bólu, ograniczenia sprawności funkcjonalnej oraz pogorszenia jakości życia. Istotną
rolę w procesie terapeutycznym odgrywa pielęgniarka, której działania obejmują edukację
zdrowotną, wsparcie psychiczne oraz monitorowanie stanu pacjenta. Celem rozdziału było
określenie roli pielęgniarki w opiece nad pacjentem z dną moczanową. Badanie przeprowadzono
metodą indywidualnego przypadku. Wykorzystano wywiad, obserwację, analizę dokumentacji
medycznej oraz pomiar parametrów życiowych. Narzędzia badawcze obejmowały
kartę opieki pielęgniarskiej, wskaźnik BMI, skalę NRS, kwestionariusz AUDIT, skalę Headley
oraz kwestionariusz oceny sposobu odżywiania. Badaniem objęto 55-letniego pacjenta chorującego
na dnę moczanową od 15 lat. U pacjenta stwierdzono otyłość I stopnia, nadciśnienie
tętnicze, silne dolegliwości bólowe oceniane na 10 punktów w skali NRS, obecność guzków
dnawych, zaburzenia snu, ograniczenie sprawności ruchowej oraz uzależnienie od alkoholu. Wykazano również niski poziom wiedzy na temat choroby, niewystarczającą motywację do
leczenia oraz nieprzestrzeganie zaleceń terapeutycznych. Na podstawie zebranych danych
sformułowano diagnozy pielęgniarskie: ból ostry, nieskuteczne samozarządzanie zdrowiem,
nieskuteczne radzenie sobie oraz bezsilność. Zaplanowano interwencje pielęgniarskie zgodne
z klasyfikacjami NANDA, NIC i NOC. Najistotniejszym problemem zdrowotnym pacjenta
był silny ból wpływający negatywnie na funkcjonowanie fizyczne i psychiczne. Uzależnienie
od alkoholu, nieprawidłowe nawyki żywieniowe oraz niski poziom motywacji utrudniały skuteczne
leczenie choroby. Kluczową rolę w opiece nad pacjentem z dną moczanową odgrywa
pielęgniarka poprzez edukację zdrowotną, wsparcie w zakresie przestrzegania zaleceń
terapeutycznych, promocję zdrowego stylu życia oraz działania ukierunkowane na poprawę
jakości życia chorego.
Gout is a chronic inflammatory joint disease associated with the deposition of sodium urate crystals in individuals with hyperuricemia. The condition leads to recurrent episodes of pain, reduced functional ability, and a diminished quality of life. Nurses play a significant role in the therapeutic process, with their activities including health education, psychological support, and monitoring of the patient’s condition. The aim of this chapter was to define the role of nurses in the care of patients with gout. The chapter was conducted using the individual case study method. Data collection involved interviews, observation, analysis of medical records, and measurement of vital signs. Research tools included a nursing care plan, BMI index, NRS scale, AUDIT questionnaire, Headley scale, and a dietary assessment questionnaire. The study included a 55-year-old patient who had been suffering from gout for 15 years. The patient was diagnosed with grade I obesity, hypertension, severe pain rated at 10 points on the NRS scale, the presence of tophus, sleep disturbances, limited mobility, and alcohol dependence. A low level of knowledge about the disease, insufficient motivation for treatment, and non-compliance with therapeutic recommendations were also demonstrated. Based on the collected data, the following nursing diagnoses were formulated: acute pain, ineffective self-management of health, ineffective coping, and helplessness. Nursing interventions were planned in accordance with the NANDA, NIC, and NOC classifications. The patient’s most significant health problem was severe pain negatively affecting physical and mental functioning. Alcohol dependence, poor dietary habits, and low motivation hindered effective treatment of the disease. The nurse plays a key role in the care of a patient with gout through health education, support in adhering to therapeutic recommendations, promotion of a healthy lifestyle, and activities aimed at improving the patient’s quality of life.
