Streszczenie
Niewydolność serca (HF) stanowi istotny problem zdrowotny i społeczny, którego częstość
występowania wzrasta wraz z wiekiem. Choroba prowadzi do pogorszenia jakości życia,
ograniczenia sprawności fizycznej oraz zwiększonego ryzyka hospitalizacji i zgonu. Kluczową
rolę w procesie terapeutycznym odgrywa kompleksowa opieka pielęgniarska oraz
przygotowanie pacjenta do samoopieki. Przedstawiono zakres opieki pielęgniarskiej nad
pacjentem z przewlekłą niewydolnością serca. Badanie przeprowadzono metodą indywidualnego
studium przypadku u 67-letniego pacjenta hospitalizowanego na oddziale kardiologicznym
z powodu zaostrzenia objawów przewlekłej niewydolności serca. Wykorzystano
analizę dokumentacji medycznej, wywiad, obserwację oraz pomiary parametrów życiowych.
Narzędziami badawczymi były: Europejska Skala Samoopieki Pacjentów z Niewydolnością
Serca (EHFScBS), Skala Akceptacji Choroby (AIS), Skala Zmęczenia Richardson oraz Ateńska
Skala Bezsenności. U pacjenta stwierdzono przewlekłą niewydolność serca wg NYHA II z obniżoną frakcją wyrzutową lewej komory (EF 30%), obrzęki kończyn dolnych, brzucha,
moszny i prącia, duszność wysiłkową oraz napadową duszność nocną. Wynik EHFScBS
(40/60 pkt) wskazywał na niepełne przestrzeganie zaleceń dotyczących samoopieki. W skali
AIS pacjent uzyskał 30/40 pkt, co świadczyło o średnim poziomie akceptacji choroby. Zmęczenie
oceniono na 7/10 pkt w skali Richardson, natomiast wynik 12/24 pkt w Ateńskiej
Skali Bezsenności wskazywał na istotne zaburzenia snu. Zidentyfikowano główne problemy
pielęgniarskie obejmujące ryzyko kolejnego incydentu sercowo-naczyniowego, duszność
i nietolerancję wysiłku, obrzęki oraz zaburzenia snu. Przewlekła niewydolność serca znacząco
wpływa na funkcjonowanie fizyczne i psychospołeczne pacjenta. Indywidualnie zaplanowana
opieka pielęgniarska, ukierunkowana na edukację zdrowotną, monitorowanie stanu chorego,
wspieranie samoopieki oraz kontrolę objawów, może przyczynić się do poprawy jakości
życia, ograniczenia liczby zaostrzeń choroby i zmniejszenia ryzyka hospitalizacji.
Heart failure (HF) is a significant health and social problem, the prevalence of which increases with age. The disease leads to a reduced quality of life, limited physical function, and an increased risk of hospitalization and death. Comprehensive nursing care and preparing the patient for self-care play a key role in the therapeutic process. Presentation of the scope of nursing care for a patient with chronic heart failure. The study was conducted as an individual case study of a 67-year-old patient hospitalized in the cardiology ward due to an exacerbation of chronic heart failure symptoms. The study utilized analysis of medical records, interviews, observation, and vital sign measurements. The research tools included: the European Heart Failure Self-Care Scale (EHFScBS), the Acceptance of Illness Scale (AIS), the Richardson Fatigue Scale, and the Athens Insomnia Scale. The patient was diagnosed with NYHA Class II chronic heart failure with reduced left ventricular ejection fraction (EF 30%), edema of the lower extremities, abdomen, scrotum, and penis, exertional dyspnea, and paroxysmal nocturnal dyspnea. The EHFScBS score (40/60 points) indicated incomplete adherence to self-care recommendations. On the AIS scale, the patient scored 30/40 points, indicating a moderate level of disease acceptance. Fatigue was rated at 7/10 on the Richardson scale, while a score of 12/24 on the Athens Insomnia Scale indicated significant sleep disturbances. The primary nursing issues identified included the risk of another cardiovascular event, dyspnea and exercise intolerance, edema, and sleep disturbances. Chronic heart failure significantly affects the patient’s physical and psychosocial functioning. Individually planned nursing care, focused on health education, monitoring the patient’s condition, supporting self-care, and symptom management, can contribute to improving quality of life, reducing the number of disease exacerbations, and lowering the risk of hospitalization.
