Rozpoznawanie i rozwiązywanie problemów pielęgnacyjnych pacjenta z reumatoidalnym zapaleniem stawów z wykorzystaniem terminów ICNP®

Abstract

Reumatoidalne zapalenie stawów to choroba autoimmunologiczna dotycząca stawów dłoni, rąk i stóp, powodująca przewlekły ból i stan zapalny w ich obrębie. Cel pracy: określenie roli i zadań pielęgniarki w rozpoznawaniu i rozwiązywaniu problemów pielęgnacyjnych pacjenta. Metody badawcze: wywiad, obserwacja, pomiar, analiza dokumentacji, skale, proces pielęgnowania i studium przypadku. Materiał badań: 87 letnia kobieta z RZS od 40 roku życia, które utrudnia samodzielne przemieszczanie, powoduje przykurcze kończyn górnych, deformacje rąk. Pacjentka porusza się z balkonikiem. W skali Barthel 40 pkt. (stan średnio ciężki), wymagana pomoc. W czasie hospitalizacji prowadzono: farmakoterapię, rehabilitację. Wnioski: Rolą pielęgniarki w rozpoznawaniu problemów pielęgnacyjnych chorej z RZS jest ocena stanu pacjentki i podejmowanie działań: diagnostycznych, opiekuńczo-terapeutycznych, rehabilitacyjnych, profilaktycznych oraz edukacyjnych. Problemy pielęgnacyjne związane z RZS utrudniają samoopiekę i samopielęgnację. Dolegliwości związane z RZS i ogólnym stanem zdrowia zdecydowanie pogarszają jakość życia pacjentki z RZS.

Rheumatoid arthritis is an autoimmune disease most often affecting the joints of the hands, arms and feet, causing chronic pain and inflammation within them. The purpose of the study: to define the role and tasks of the nurse in recognizing and solving the problems of nursing problems of the patient based on the assumptions of the International Classification of Nursing Practice (ICNP ®). Research methods: interview, observation, measurement, documentation analysis, scales, process of of nursing and case study. Findings: An 87-year-old woman with RA since the age of 40, which hindered independent movement, caused contractures of the upper limbs , deformities of the hands. The patient moved with a walker. On the Barthel scale, 40 points. (moderately severe condition), assistance required. During hospitalization was conducted: pharmacotherapy, rehabilitation. Conclusions: The role of the nurse in recognizing the nursing problems of a patient with RA is to assess the patient’s condition and to take measures: diagnostic, care-therapeutic, rehabilitation, prevention and education. Nursing problems associated with RA, make self-care and self-care difficult. RA-related complaints and general health conditions definitely worsen the quality of life of the RA patient.
