RekordAPOE as a risk factor for Alzheimer’s disease and lipid metabolism disorders: molecular analysis(Akademia Bialska im. Jana Pawła II, 2024-05-08) Kowalska, Ewa ; Madej, Łukasz ; Szafraniec, Kacper ; Rachuna, JarosławThe gene encoding apolipoprotein E (APOE) plays a key role in many biological processes, including lipid metabolism, brain homeostasis, and functioning of the circulatory system. APOE gene polymorphism is one of the most important risk factors in the context of Alzheimer’s disease (AD), which is the most common cause of dementia. Different variants of APOE alleles, particularly APOE ε4, are associated with an increased risk of AD and accelerated progression of this disease. Mechanisms by which APOE ε4 contributes to AD pathogenesis include amyloid metabolism disorders, tautopenias, immune system dysfunction, and functional abnormalities of the blood-brain barrier. The APOE gene may play an important role not only in the pathogenesis of AD, but also in the development of cardiovascular diseases, such as coronary heart disease, stroke and atherosclerosis. As in AD, different variants of APOE alleles may affect lipid metabolism, inflammation, vascular functions and the processes of apoptosis, which translates into an increased risk of diseases. The article presents the current state of knowledge about the role and importance of the APOE gene in the pathomechanism of AD and cardiovascular diseases. Understanding molecular mechanisms and gene-environment interactions associated with APOE may contribute to changes in the diagnostic process. The development and improvement of genetic tests may help in early detection and treatment of diseases, and even, in some cases, prevent their occurrence.

RekordAssessing the impact of home-based conventional exercise and cryotherapy on lower limb muscle strength and walking ability in individuals with knee osteoarthritis: a randomized controlled clinical trial(Akademia Bialska im. Jana Pawła II, 2024-05-17) MohammedSadiq, Hawar Abdulrazaq ; Rasool, Mohammad TahirBackground. K nee osteoarthritis ( KOA) i s a c ommon a ge-related joint c ondition c ausing discomfort, reduced physical capability, functional diminishment, and diminished life quality. Material and methods. I n t his c linical r andomized c ontrolled t rial, K OA-diagnosed individuals were randomly allocated into three cohorts: an experimental cohort (n=17), a control cohort (n=14), and a secondary control cohort (n=17). Interventions included two months of home-based exercise (HBE) with cryotherapy, exclusive HBE engagement for two months, and routine therapeutic and physiotherapeutic interventions at a medical facility. Results. The experimental and first control cohorts exhibited notably enhanced lower-limb muscle strength and walking capabilities compared with the second control cohort after two months. Both experimental and first control groups showed significantly lower fivetime sit-to-stand scores (5TSTS) and walking 40 meters scores compared with the second control group (p<0.0001). However, the control group displayed slightly improved scores in 5TSTS and 4x10-meter walking relative to the experimental group, though not statistically significant. Conclusions. Combining HBE with cryotherapy did not significantly improve lower-limb muscle strength and walking in KOA patients.

RekordBarriers and facilitators for early and exclusive breastfeeding in the Duhok Governorate in Iraqi Kurdistan(Akademia Bialska im. Jana Pawła II, 2024-04-18) Haleem, Khaled Hussein ; Mohammed, Bashar Ibrahim ; Abdulrahman, Nareen Arif ; Haleem, Azad Abduljabar ; Atrushi, Akrem MohammadBackground. Breast milk is the best f ood satisfying a baby’s needs. Sociodemographic factors, antenatal care practices, and professional assistance at birth may be barriers against this. The aim of the study was to estimate the prevalence of exclusivity and to identify the barriers to breastfeeding. Material and methods. A cross-sectional study was conducted from August 2022-2023 involving 336 mothers of infants (6-24 months) who visited primary healthcare centers. Information on socioeconomic background, pregnancy and delivery details, and awareness of breastfeeding benefits was collected using a pretested questionnaire. Data analysis was performed and a p-value ≤ 0.05 was considered significant. Results. The whole number of infants breastfed was 146 (43.45%), and for six months, this was 25 (7.4%). The significant factors were: living in Duhok City, not having sufficient knowledge about the breastfeeding, decision to breastfeed during pregnancy, starting breastfeeding soon after delivery, vaginal delivery, order of birth, not giving sugar after delivery, being supported by the husband, holding the baby after delivery, knowledge of benefits of breastfeeding. The most significant factors associated with exclusive breastfeeding were maternal age <25 years, diseases during pregnancy, birth order (2nd o r 3rd ), explanation of the benefits of breastfeeding by health professionals, decision to exclusively breastfeed during pregnancy, delivery in a governmental hospital, and holding the baby after birth (OR=1.09, 1.037, 1.048, 1.677, 5.869, 1.097, and 2.45, respectively). Conclusions. This study identified multifaceted barriers to early exclusive breastfeeding in the Duhok Governorate.

RekordEffect of obesity on the relationship between glycosylated hemoglobin and insulin resistance in young adults(Akademia Bialska im. Jana Pawła II, 2024-05-17) Topal, Sümeyra ; Yalnızoğlu Çaka, Sinem ; Çınar, Nursan ; Altınkaynak, SevinBackground. The purpose of this prospective case-control research was to compare insulin resistance in obese and non-obese students with regard to glycosylated hemoglobin (HbA1c) and the homeostatic model assessment insulin resistance (HOMA-IR). Material and methods. This study was conducted with a total of 122 students who were determined to be obese (n=35), overweight (n=43) and normal weight (n=44), among students at a university in Türkiye (n=891). Students were first classified according to their Body Mass Index (BMI) and blood samples were taken for fasting blood glucose, HgA1c, and insulin levels. Results. Of the 891 students participating in the study, 4.83% (n=43) were overweight and 3.93% (n=35) were obese. HbA1c levels were found below 5.7 for 72.7% of normal weight students and HbA1c levels were found between 5.7-6.4 for 65.7% of obese students. There was a significant statistical difference between them (p=0.001). When the HOMA-IR value was >3.16, there was a significant difference between the obese group and the other groups (p=0.042). There was a significant positive correlation between BMI and both HbA1c (r=0.299, p=0.001) and HOMA-IR (r=0.432, p=0.000). Conclusions. It was found that HOMA-IR was more common in obese students, and early diagnosis of insulin resistance might be HbA1c. HbA1c can be used since it is inexpensive and simple to evaluate insulin resistance in obese young adults.

RekordInvestigation of GST and drug resistance protein expressions in relation to chemotherapy in breast cancer(Akademia Bialska im. Jana Pawła II, 2024-05-08) Kaya Kocdogan, Arzu ; Kilic, Murat ; Oguztuzun, Serpil ; Benzer, Emine ; Dilek, Gulay ; Kahraman, Yavuz Selim ; Gulcelik, Mehmet AliBackground. During cancer therapy, resistance to chemotherapeutic agents is common and often linked to multidrug resistance mechanisms. Glutathione S-transferases (GSTs) and ATP-binding cassette (ABC) transporters, including multidrug resistance (MDR) proteins, multidrug resistance-associated proteins (MRPs), and breast cancer resistance protein (BCRP), are implicated in drug resistance. Material and methods. Protein expressions were determined in the tumor and surrounding tumor free breast tissues of 145 breast cancer patients by immunohistochemistry technique. Among 145 patients, 50 received preoperative (neoadjuvant) chemotherapy, and 95 received postoperative (adjuvant) chemotherapy. Results. In 50 neoadjuvant breast cancer patients, GSTA1, GSTK1, GSTM1, GSTO1, GSTP1, GSTS1, GSTT1, GSTZ1, MDR1, MRP1, MRP2, MRP3, MRP7, and BCRP expressions were higher in tumor epithelium compared to normal epithelium (p<0.05). In 95 adjuvant breast cancer patients, GSTA1, GSTK1, GSTM1, GSTP1, GSTT1, GSTZ1, MDR1, MRP1, MRP2, and MRP3 expressions were higher in tumor epithelium than in normal epithelium (p<0.05). GSTP1, GSTT1, and MRP3 expressions were significantly higher in neoadjuvant compared to adjuvant-treated breast cancer patients’ tumor tissues (p<0.05). Conclusions. GSTP1, GSTT1, and MRP3 may be important in inactivating the chemotherapeutic agents used in platinum-based treatment and are thus responsible for the drug resistance in breast cancer patients.

RekordPentalogy of Cantrell: a case report(Akademia Bialska im. Jana Pawła II, 2024-05-17) Płusajska, Justyna ; Świercz, Grzegorz ; Młodawski, Jakub ; Malmur, Mariusz ; Zalega, MagdalenaCantrell’s pentalogy is a rare developmental disorder occurring between the 14th and 18th day of embryonic life. Diagnosis is possible in the first trimester, when the rupturing of the continuity of the anterior chest cavity and abdominal walls is usually found. Ectopic heart and umbilical hernia is also characteristic. In this article, a case of a thirty-three-year-old patient in second pregnancy was described, who was diagnosed with developmental disorders during a routine visit to a gynecologist. Woman was referred to the Laboratory of Prenatal Research, where the fetus was diagnosed with: umbilical cord hernia, diaphragmatic hernia with diaphragmatic defect, sternal defect, ectopic heart and pericardial defect, the swelling of the entire fetus – NT 5 mm – and the reverse flow in the DV were visualized. The woman and the child’s father, after having been presented with the prognosis, decided to terminate the pregnancy. Due to the heterogeneous picture of disorders occurring in pentalogy of Cantrell and the small number of cases described in the literature, each case should be approached with caution and in an individual manner. Abnormalities in the structure of the heart additionally compounded by coexisting disorders are crucial for the prognosis and they determine the method and order of treatment.
